Information We Collect We may collect the following categories of personal data: 1.Clinician Information •Name, professional title, and contact details •Account login credentials (where applicable) •Clinical workplace or affiliation 2.Patient-Related Data (entered or dictated by clinicians) •Consultation notes, discharge summaries, referral letters, clinic letters, and MDT summaries •Voice-to-text transcription data •Patient identifiers where applicable (e.g., name, ID number, date of birth) Important: HEMI AI Health does not independently collect patient data. All patient information is provided by clinicians as part of clinical documentation. 3.Technical and Usage Data •Device information (model, operating system, browser type) •Log data (IP address, access time, app interactions) •Cookies and similar technologies (for website hemihealth.ai only) 2. How We Use Your Information We use personal data to: •Provide, operate, and improve HEMI AI Health •Enable AI-powered transcription and document generation •Support clinical decision-making with knowledge tools •Maintain data accuracy, security, and compliance •Provide user support and respond to enquiries •Conduct research and development (in anonymised or aggregated form) 3. Legal Basis for Processing We process personal data based on: •Consent – when you choose to use HEMI AI Health •Contractual necessity – to deliver app functionality •Legitimate interests – to improve services and maintain security •Legal obligations – to comply with applicable laws and regulations 4. Data Storage & Security •All data is encrypted in transit and at rest. •Servers are hosted in secure environments with industry-standard safeguards. •Access to data is restricted to authorised personnel only. •Where possible, data is anonymised or pseudonymised. For users in the UK, data may be transferred outside the UK in compliance with UK GDPR safeguards (e.g., Standard Contractual Clauses). 5. Clinicians may request deletion of their data in accordance with local legal requirements. 6. Disclosure of Information We do not sell or trade personal data. Terms of Use HEMI AI Health – Your AI Partner in Clinical Excellence Effective Date: 11 September 2025 Last Updated: 11 September 2025 These Terms of Use (“Terms”) govern your access to and use of HEMI AI Health (the “App”), developed and operated by MedPlanner Sdn Bhd (“MedPlanner”, “we”, “our”, or “us”). By downloading, accessing, or using the App, you agree to be bound by these Terms. If you do not agree, please do not use the App. 1. Eligibility HEMI AI Health is intended for licensed healthcare professionals and clinical staff. By using the App, you confirm that: •You are a clinician or healthcare professional authorised to handle patient information. •You will use the App only in compliance with applicable laws and professional standards in Malaysia, the UK, or your jurisdiction. 2. Purpose of the App HEMI AI Health provides AI-powered tools, including: •Voice-to-text transcription for clinical documentation •Automated generation of consultation notes, discharge summaries, referral letters, and MDT reports •Clinical knowledge assistance Disclaimer: HEMI AI Health is not a substitute for professional clinical judgment. The App provides support tools but final responsibility for diagnosis, treatment, and patient care rests with the clinician. 3. User Responsibilities When using HEMI AI Health, you agree to: •Ensure that any patient data entered is lawful, accurate, and handled in line with your professional duties. •Obtain necessary consent from patients where required. •Not misuse the App for unlawful, unethical, or unauthorised purposes. •Maintain confidentiality of your login credentials and account. 4. Service Availability •We strive to provide continuous availability but do not guarantee that the App will always be uninterrupted or error-free. •We may update, suspend, or discontinue features at any time to improve service. 7. Limitation of Liability To the maximum extent permitted by law: •MedPlanner is not liable for any indirect, incidental, or consequential damages arising from use of the App. •Clinicians remain responsible for all clinical decisions and patient care. •The App is provided “as is”, without warranties of accuracy, completeness, or fitness for a particular purpose. 8. Termination We reserve the right to suspend or terminate your access to the App if you violate these Terms or misuse the service. 9. Governing Law •For users in Malaysia, these Terms are governed by the laws of Malaysia. •For users in the UK, these Terms are governed by the laws of England and Wales. Any disputes shall be subject to the exclusive jurisdiction of the courts in the relevant country. 10. Changes to Terms We may update these Terms from time to time. Continued use of the App after updates constitutes acceptance of the revised Terms. 11.
